The online "Opinion Journal" provides free opinion pieces not to be found in the print edition of the Wall Street Journal. Today's OJ features a piece by a M.D. defending Single Payer Health Care . It's quite persuasive. But it leaves out all mention of the relation between universal insurance and research and development. What does that mean? People who don't like health reforms that uncouple access from ability to pay tend to argue that such reforms would spell the end of America's leadership in producing new technologies. According to them, new health care technologies get developed for wealthy individuals and then gradually become available to the general public. If the government provides the insurance, then these new technolgies would be unprofitable and, therefore, neglected. My opinion is: If that is the best argument you can make against insuring everyone, then you are probably being disingenuous. Surely we could find some other way to support appropriate R&D. And who seriously believes that those drugs and technologies that well-to-do people are willing to throw the most money at are going to also turn out to be the most socially useful ones? Viagra anyone? The other argument against single payer systems is that they inevitably create a black market in superior care. Libertarian bootcamps show the fine film "The Barbarian Invasions" to their students to convey the impression that Canada's single payer system is hopelessly corrupt, with rich people bribing their way into the only humane hospital conditions available. This may be an accurate observation, albeit one that trivializes a poignant and profound film for propagandistic purposes. Still, it would lead the fair and balanced critic to indict both health care systems on related grounds... rather than view one as unambiguously better than the other. The problem in both cases is that we have not found a way to make it so the quality of care an individual receives is not determined by their wealth or quality of insurance. I'm not myself a defender of single payer systems. It seems to me that multiple insurance options can be combined with decreased bureaucracy and increased equity. But this is a very interesting and persuasive op-ed.

Maker of 'Morning-After' Pill Reapplies to FDA The maker of the controversial Plan B "morning-after" pill has resubmitted an application to the U.S. Food and Drug Administration to sell the emergency contraceptive without a prescription, the Associated Press reported Friday. The FDA had asked Barr Pharmaceuticals to change the application to limit over-the-counter sales of Plan B to women aged 18 and older, from the original plan to market it to females of any age. Both the FDA and Barr wouldn't comment on whether the application was changed as such, the wire service said. Plan B is now available in most states only by prescription. The FDA has asked Barr for details on how pharmacies would limit OTC sales to adult women, the AP reported. U.S. Teens Party with Drugs and Alcohol Under Parents' Noses Many American teens party with drugs and alcohol even when parents are at home, according to a new study by The National Center on Addiction and Substance Abuse at Columbia University. The survey included 1,297 young people, aged 12 to 17. Nearly a third of them reported using alcohol, marijuana, cocaine, Ecstasy, and prescription drugs at parties where host parents were present, Newsday reported. Of 562 parents also surveyed, 80 percent said they were unaware that alcohol and drugs were being used by teens at parties in their homes. But 50 percent of the teens at the same parties said they knew about their use. "That shows just how out of touch the parents are," Joseph A. Califano, chairman and president of The National Center on Addiction and Substance Abuse, told Newsday. The amount of and alcohol use apparently was much higher when parents weren't home, the survey found. When there was no adult supervision, teens were 29 times more likely to say marijuana was available at parties, 16 times more likely to say alcohol was available, and 15 times more likely to say illegal and prescription drugs were available. Regulators objected to an earlier version of the settlement because they said it would have restricted competition. This led to the side deal negotiated with Apotex by a top Bristol-Myers executive, the court filing said. Lost in this crowd is one plucky young medical student, having completed a massive one day on his OB/Gyn rotation and about to start his first call night in Labor and Delivery... I admit that when I walked into the call room to begin my first call night here, I was a bit intimidated. Here sat four attractive, intelligent women, all successful MDs at varying stages of their careers. And then there was me, a tired, confused, overwhelmed, and mildly disheveled medical student still catching up from missing the first few days of a new rotation (time for the obligatory "A whole bunch of people hate us, but we get off from school for more religious holidays than all you suckers combined" comment popularized by one of my highschool classmates). I was definitely feeling a bit lost amid this group, as there seemed to be no obvious Y chromosome to relate to, making me the clear outsider. Would they reject me? Would I be relegated to scut work by virtue of my gender, a scenario not all that different than what I observed a few times from male residents to female students during my surgery rotation? Would any of these women go out with me? Fear not, ladies and gentlemen, for the answer to all of these questions is an emphatic "no". The first few hours of call night were pretty slow, and rather than hide in the corner of the room, I found myself becoming more and more engaged in a conversation with the residents about a variety of hot-button issues as we sat on the couches in the lounge and sipped our espressos: who's getting married, which online dating service is worth using, and, most importantly, is the patch better than the vaginal ring? Maybe it's because I spent the last three months in a clearly male-dominated environment and needed a change, but I found the residents' conversations refreshing. Before I knew it, I was totally getting into this conversation. Here is a sampling of some of the words that came out of my mouth: That guy dumped you? He's a fool! She slept with both of them? Dirty ho! That's the best excuse he could come up with? Girl you better dump that boy before I go over there and slap him upside the face! Let's watch Access Hollywood. Isn't Noah Wylie just so dreamy? Why aren't there any doctors like him around here? Don't mess with that nurse, she's on her period! What's the next book for Oprah's book club? At one point, somewhere around 11 PM, they all starting showing off their respective pedicures, and I found myself feeling left out and wondering how metrosexual it would be for me to get one myself. Then they all started braiding each other's hair. I shit you not. Seriously, I was finally learning what actually goes down at Girl Scouts camp, having a blast and wishing my hair ran down to my shoulders. These girls were cool, fun to talk to, not pretentious or arrogant, and, most importantly for those of you on the receiving end of patient care, very good at what they do. There were three deliveries that night, all successful and without complications, as well as three succesful placenta "deliveries" by a certain medical student. (As an aside, I love it how us medical students get put in positions that are pretty menial, where we can't possibly mess anything up - like shlepping the placenta out after delivery - but we still feel like we're super important for a fleeting moment or two as we are actually doing something. Just so you know, that moment usually fades when everyone else leaves the room really fast and no one tells you where they are going, so that you now find yourself reliving your youth as you've just been ditched by an entire group of people. Not that that has ever happened to me before or anything.) In all, it was a great night. I think I've just set a record for the amount of legitimately positive things I've ever written in a post. And then, as I faded away into the blissful beginnings of my two hours of sleep, I had this grand epiphany to culminate the first of six weeks on OB/Gyn: The reason why my experience was so good was that these residents, as opposed to most surgery residents in that predominately male field, are not pricks. And the reason why they are not pricks is because...well...they don't have any. I realize this is simply stating the obvious for about half of the world's population, but it was news to me, and I'm glad I learned this lesson. Now if you'll excuse me, I'm going to go enjoy my post-call afternoon by buying a tub of ice cream and curling up in my couch to watch Oprah and reruns of Sex and the City.

Its very convincing as well. Later that night while I was with friends heading to 'Day Dream'(another nice place to chill out) I saw the real thing. I can't believe my eyes that it would look so grand. From what I expect is totally different. Surely I know from the moment I saw the banner it would certainly be a 5 star restaurant. Never like what i imagine. Anyway, I got so 'sakai' n stop my car beside it. Took so many pics. It was marvelous. The interior and exterior was beyond creative and beautiful. Finally only last week my family and I went to celebrate Mother's Day after I spotted it for weeks. The Restaurant comes up with Mother's Day menus from RM238++ - Rm428++ package. There are 3 packages offered. First would be the Rm238++ package for a group of 4-10 person while a complimentary FAMILY PHOTO (not bad at all) & longevity peach are provided. 2nd package would be RM338++ for 6 person. Lastly Rm498++ for 8-10 person. That's the nice part of it. But I personally have NEGATIVE reviews to it. It was a sunny that day. It got a little warm in there. Seems that theres no air-conditional at all. It's covered with glass. Almost half of it at least. With no curtains at all. The sunlight got straight in. Disastrous in the morning but it definitely is superb at night. That's the heat issue. Next would be the service. We took up the Mother's Day package. So, they ask us to reach at 1230pm. Ok then. we reach a little earlier, 1220pm. Waited and waited. 1235pm...mmm...ok. Chit chat our way through. 1240pm...still fine. 10 minutes had past. Tummy growling like hell but put on a nice show...smile...1245pm...we counted our minutes and that's as long as we can wait. Waited for damn 15 freaking minutes. So my mom ask one of the CHINESE waitress to check if our food is done yet. Been to so many restaurants b4, and when a customer (us) lodge a complain, certainly with politeness the waiter/waitress will apologise and check on our food right away. This was the different case. Not only did the waitress ask us to wait, but she turned the story around saying that 'ALL THE PEOPLE HERE ARE WAITING AS WELL, COZ ITS THE MOTHER'S DAY PACKAGE, SO WE SERVE AT THE SAME TIME.' WTF ...whats that about??? Thats a little too rude. Sounds more like 'CAN YOU WAIT AND SHUT JE MOUTH. I'M ALREADY HAVING AN HEADACHE WITH ALL THE COMPLAINTS AROUND.' A good restaurant won't let their customers waiting. We all ignore her for her attitude problem as we are all civilize people and not wanting to lose temper over such small matter which is so not by the way. Impression down. Atlas hurray the food arrive 5 minutes later. Lucky it was 5 and not 10. My dad surely will get grumpier. SO....wheres the chopstick...they serve us food with plates and bowls. What are they expecting us to do with those?!? I have no idea!!! So we kindly approach an Iban lady to have our chopsticks. She APOLOGIZE and flashed off. WoW!!! At least this won't give waitresses a bad name. Then each person brought a set of chopsticks to us. Weird huh. Why can't ONE person does it all??? I kept wondering. Still wondering now. BUT!!! I'm the only one not having a set of chopstick. AGAIN calling out for them. Man...that was frustrating enough. Customers are always right. There...I said what I wanted to say. That's what I expect from a 5 star restaurant. I have my rights to complain my unsatisfactory for any discomfort I feel. I manage to take some pics while waiting for the food to be served. My stupid brother took my pic when i was making fun of my face. I was imitating girl's famous cute pose. But I did something else instead. Nobody wanted to take my pic. I had no choice but to take my reflection from the mirror. Not the original me. Another poser in the family.

I am ready for some meaningful football. (My fiancee is now rolling her eyes over the last two words in that sentence... but still). Tonight sees the Oakland Raiders visit the New England Patriots at Foxboro for the kick-off game of the Pats' quest for three straight. I see this as a reasonable objective for the Patriots, but it will, as always be a difficult journey. I see the Patriots as a well managed team that attempts to use its versatility as its core competence. As I have written before, the Patriots seek to take away the opposition's first choice option. The fundamental gamble here is that the Patriots are betting that their second, third or fourth preferred style of play is vastly superior to whatever the opponent's second preferred style of play. This was seen most notably in the 2002 Super Bowl against the Rams where the Patriots were able to disrupt Marshall Faulk for the entire game, and thus putting the offense solely on Kurt Warner's shoulders. The Greatest Show on Turf managed 17 points. This year, I think that the Patriots are significantly deeper and more versatile on offense. With the return of Ben Watson, I will be shocked if the Patriots do not line up with two tight ends for at least forty percent of their first downs. Dan Graham and Ben Watson present some intriguing passing game match-up problems for any team that does not have multiple rover linebackers/strong safeties (6-2+, 225lbs+, 4.5 or better speed) and if a team has a light fast defense in, the tight ends can stay in and assist the power running game. This is one of the many less than pleasent choices the Patriots force teams to make. I would think tonight that the Patriots would want to take advantage of the comparative slowness of Oakland's big linebacker corps and run plenty of crossing routes and seam patterns with receivers and tight ends isolated against a linebacker who is bigger than some defensive ends. Now onto defense, this has been the forte of the Patriots' ability to mirror and deny an opponent's strength over the past four years. I am liking the defensive line quite a bit. I was surprised that the Patriots cut Rodney Bailey in favor of keeping undrafted rookie Mike Wright, but even still, the Pats in their base 3-4 have significant talent in their starters and impressive depth. Richardy Seymour and Vince Wilfork each demand two blockers on running plays, and Ty Warren will dominate against most right tackles when he is one on one. Oakland will be forced to keep their running backs or tight ends in to help block more often than they would prefer. The strength of the defensive line is the strength of the unit as a whole. If the three linemen and the typical pass rushing linebacker can force Oakland to keep seven blockers in, the task for the secondary just got a whole lot easier. I am not sure who the starting cornerbacks will be tonight, as the Patriots currently have four corners who have started for Super Bowl winners on the roster. I would imagine that the Patriots will attempt to play some aggressive man with zone support against Randy Moss and keep Chad Scott in as the physical bump and run corner for at least third down plays. I also anticipate the Patriots keeping Eugene Wilson in deep centerfield for most of the game, as I think that the Patriots would be willing to see Moss have a T-O type game if they can shut down everyone else on the field. They just need to avoid the multiple big plays that are Moss's bread and butter, even if that means giving up more 10-15 yard gains. I am not sure what the Patriot linebackers will be doing this year. The outside linebackers are experienced, fast, smart and solid. Roosevelt Colvin should finally be completely healed from his hip injury suffered two years ago while Willie McGuinest just gets craftier and better as his health is still with him. I doubt that Willie McGuinest will be healthy for the entire season, but Tully Banta Cain, in limited playing time, has shown some significant pass rush ability over the past year. Mike Vrabel is just a playmaker who is never out of position. The loss of the top three playing time inside linebackers from last year is a large loss which creates a significant question mark. I am glad that Vrabel is shifting to the inside, as he is smart, and it will keep the Pats' three best linebackers on the field more often. 'Oops' I said quietly. The crowd gasped. 'Do not worry' I whispered. 'The larva must emerge as part of its natural life history. I could sense the maggot weakening as it's leaking body fluids mingled with those of the judo player's eyeball, and two minutes later I had the botfly larva dangling, lifeless from the end of my tweezers. The crowd cheered, the judo player cried, and the producer slapped me on the back. 'Thank Christ for that... she cheered, smiling for the first time since the shoot. 'This is going to send the ratings rocketing. Did you get all that Chris?' I turned around to see a tall man bending in my direction. He was holding a steady cam, which was currently pointed at my face. 'And......Cut!' shouted the producer. Ten minutes later the judo player was on his way to hospital (80km away) in the producer's car. I was kept behind,at the producers insistence, to do a piece to camera . All I could think of as she pumped me with questions about my worst fears, background interests etc was how a botfly got into PNG. So, dear reader, was this how I became a celebrity? Nope. We still had a week to go and we were down to four celebs. What I didn't know then was what the producer had in store for yours truly.
