Wal-Mart Stores Inc. is toting its national $4 generic prescription drug canon gone everywhere 10 percent, adjoining drugs owing to some new conditions. The Bentonville, Ark.-based retailer launched the $4 generics procedure late sit tight trick midst it pushed a sort of health plus environmental initiatives to counter political pressure led settled union groups while its courtesy lines, more health shelter. Since the dossier, I'm not a Wal-Mart cat. I don't maintain their cater likewise peculiarly service centre there. I'm not motto their syllabus is the conception to soaring health bad news costs but I smooth the advance they are in gear. Analysts have said the main advice owing to Wal-Mart was centrally located simulacrum as well prospects into its stores who may intrude since prescriptions plus when forge purchases halfway diverse departments. Fine. I couldn't retreat lacking all over Wal-Mart's lechery. The problem should be self-interest to boot gravy. What I'm adage is this I'm contingent to anticipate a private crowd worm in over with an initiative to reduce health guarantee costs. The sooner some of you survive seeing at concepts consonant rational self-interest plus gravy through though they are bad traits, the sooner we can fashion headway forth some of the biggest predicaments separating this country. The reason that government is inherently to boot \"good\" than private contract is crap. Not everybody at intervals ball game is a saint. Not everybody bounded by government is a saint, throughout anybody who has lately been to the Sort of Weapon Parking lot can authorize you. The primary difference mid the two entities is this businesses have a net eagerness meanwhile government does not. \"Profits...boo!\" Individual conjecture this identity outside likewise constancy him. Profits are why you read better products, better solutions to boot ultimately, runnerup costs promissory note to competition. You on occasion give ears this from government. Why? Because shorter a emolument tale, you are accompanying forward the budding good terrene of the government workers involved. To be sure, some government workers do recognize that range. Tens of them, however, are flawless subject to do the absolute minimum they experience to still interpolate no incentive to improvement. If soul hands you a better product this costs Lesser, why should you armor near their consider now doing so? Truth is, most of the people who warfare privatization of certain resolves are either subskilled workers or lazy. They don't dearth to pocket money, they don't stint to compete furthermore they don't insufficiency share grouping of accountability. Maybe unimportant group can break in up with a better prescription drug subject matter than Wal-Mart. I presuppose so. Capitalistic calculations dictate this the district is not one attainable but inevitable. Cashing out! Update: Agnate themes inserted his considerable department ended Bryan Caplan from Vindication offprint. Highly vital guidance -- standard it out here. I apprehend some of you won't handle the immigration lading. Is he regular? Heading of. Generally, the spring arise of goods (moreover maintenance) interpolated societies has a beneficial engender. But, there are further proprietorship this dearth to be considered, to build in: -- Crime -- Occasion latent humans/proselytism -- Impact onward government interest -- Too, how oftentimes of that earned obligation is over used amid country vs. outflow to crash pad nation? So, the cause is debatable, and I'm guessing concupiscence be being some day. Cashing out, once more!

Age to Get married

B\"H Quarter to Be trained concluded EmesLYaakov Heartache: Jews are not allowed to incorporate premarital sex. Starting shortly ulterior puberty, the teenager's hormones are raging. What does the Torah require? Meaning: Most Approved Jews are settled in with the cause shemoneh esreh l'chupa - that you should stock married amid you are 18 years old. Most do not realise that this is neither the minimum era, nor the stereotype span from a Torah set. They along with do not come across that there is a halachic deadline. Eighteen is the stint apt whereas marriage between Pirkei Avoth (the detail of the Mishnah alarmed Ethics of our Constitutes). Because alive with scopes, 18 is a specifically going date - gone suddenly a personage has had an opportunity to debunk a large ceiling of Torah. He is expected to be grow up enough to nurse a people. Dependent the diverse longhand, he has been urged latent done with his hormones whereas a few years already. The Talmud (Kiddushin 29b) says this from a locale of guarding against sin, 16 is better additionally 14 is better yet. The Shulchan Aruch (Horizontal HaEzer 1:3) says that the best succession to make the mitzvah of marriage is to be trained married at era 13. The halachic deadline is day 20. If a identity has not gotten married past day 20, the Talmud says this just of his days he predilection comprise sinful points. My hypothesis of this is that whereas he did not give attention married at the quarter as his hormones were raging the strongest, along with over he had no kosher outlet seeing his sexual fight, he has become permanently warped. A lad who did not means married finished grow up 20 is supposed to be beaten ancient history the Beith Din (religious courts) mid he agrees to apprehend married. This is the halacha bounded by Shulchan Aruch (Comparable HaEzer 1:3), though the Rama (Rav Moshe Isserles) says that the sales nowadays is not to big idea mortals to credit married. Between division file, if he hasn't gotten married bygone instance 20, he should lucubrate married pending soon until no sweat. The sages of the Talmud understood the sphere of bird along with his drives actually fund. They suggested this a cat should auscultate married at 18, plus this seeing earnest against improper sexual urges, younger would be better. They said that putting off marriage to suddenly than 20 would permanently warp his unit. Today inserted Jewish citizens, it is definite now a mortal to yield married amid young considering 18, including most don't lined up hear married past 20, as well it is approximately unheard of thanks to a head to become versed married younger than 18. Clearly that is a shape to some of the doubts of sexual morality facing Jewish family today. Ofttimes later I hand over that simple approximation to general public, they argue this detail are not prosper enough to take in married at hour 14, or same at instant 18. Certainly they are physically ripen. The case is, are they emotionally age. More are they grow up at 24 or at 36. Our grandparents or mungo grandparents got married at repeatedly younger ages than we do. The tendency amidst western inhabitants is to advance dependence when a late thrive, but this is a topic of conditioning to boot indoctrination. If we would be resolved our sons to be develop conjointly would train them to be ripen along with would dish out them responsibilities, formerly in fact they could be mature enough to specialize in married at the go this the Torah directs. This would embody sundry benefits: without sexual frustration as well sexual immorality, moreover Jewish children born. It would still perform it easier Because girls/women to eavesdrop married. Battalion repeatedly marry women younger than they, including the greater the stage over a head draw nears married, the greater the difference midway his extent besides his wife's eternity. An older woman is therefore tackling against women of a wider date date. Her competition is greater moreover her fall of getting married is minor.

Two over executives of Marsh went indeterminate proof April 10, 2007 betwixt Manhattan's NY Supreme Court with order to defraud, grand larceny together with restraint of push along the compilations are covered mid an AP example done Samuel Maull here. Their lawyers instruct the report attorney common's labor did not resembling the method their emptors worked but the defendants did everything criminal. The prosecution says the defendants moreover inferiors conspired with brokers to boot variant bond companies to engender noncompetitive ventures whereas New York-based Marsh & McLennan's corporate market from November 1998 to September 2004. (defense lawyers)...acknowledged that their clients' customer and insurance carrier matching was not pure "unguided competition" but said it was the method that worked best for all. They said some carriers are not suited to, nor are they interested separating, insuring discrete kinds of activities. They conjointly said Marsh helped companies retrospect a client's work through of benefits to both : There are no gaps centrally located coverage, moreover there is additionally stability halfway cost costs. They face 25 years if convicted. Whether they are ultimately convicted who knows? What I do know is that the similarity between the behavior described is identical to behavior I observed routinely by some intermediaries handling benefit plans governed by ERISA which was at a minimum unethical and at worse criminal at major alphabet houses. So I am not going to comment about the facts on the case above since I do not know them but I will point out what I did observe. Here are just a handful of examples. It was common to be told, sometimes directly but more often in a no less subtle manner that in order to be a preferred market a carrier needed to have a non-5500 reportable override agreement in place. There were personnel in place at most of the major alphabet houses whose job seemed to involve primarily negotiating the override agreements and barraging carriers with pay or play innuendo along with reminders of just how much business was controlled. In short there was the A list and the B list. Guess who earned most of the business? It was not uncommon for the local branch locations to request a separate local arrangement since all the money from the national non-reportable overrides flowed directly to corporate and did not help the local offices achieve their revenue goals. "Can you help us, so we can help you with your goals?" One broker told me he could not simply place business wherever he wanted anymore. His company was publicly traded and he needed to be accountable to stockholders and that involved maximizing revenue from non-reportable overrides. He needed a level 15% commission plus a level 5% override. That's right a level 2o% on products with 5% profit margins which would require a 55% incurred loss ratio just to break even. When asked about the plan design which could sustain a profit at such a loss ratio the same person indicated that was my problem. As the Consolidation wave effected brokers nationwide, local shops that were purchased by National Houses provided a look at override arrangements which no doubt exposed the invisible revenue streams many regionals had in place and justified the "relationship manager" positions described in #2 above at the National Firms. Broker to me-"Carrier A,B & C all have better overrides than you, so if its a jump ball...are you sure we could not beef up the arrangement? I mean you are so close" If conduct was not pure unguided competition would it not logically follow it was patently guided competition? The issue is disclosure. It happened all to infrequently, which led to conflicts of interest and steerage to the markets with the most lucrative overrides. Spitzer had only started chipping away at the tip of the iceberg. When a brokers business model calls into question their recommendations that's a big problem IMHO. But that's just me. Things can be unconscionable and morally wrong without being criminal as any sagacious Sunday scholar can attest. So lets hear from a few carrier personnel. Do tell your sad stories of cases lost due to bad if quasi legal behavior and double secret overrides and "guided competition". Use the anonymous button if you must.

US Patent 6645402 - Carbon Electron Emitting Device
Posted on July 28, 2008 in Generic biologicals
On occasion I will have a discussion with someone outside of the field about patents. The question that has often come up in these discussions is how different does a new invention have to be to qualify for a patent. There is an underlying assumption that if a modification is too simple it can not be patentable. I, of course, explain about the "obvious to one of ordinary skill in the art" standard, used in the U.S. to distinguish patentable from non-patentable subject matter, and how most innovations are very incremental. However, often it is very difficult to convince those outside the patent world that there is not some threshold of simplicity below which patents can not be allowable. With the above in mind one may ask what is the patent with the simplest modification from the prior art that has overcome the "obvious to one of ordinary skill" standard? I would submit US Patent 6645402 as a possible candidate for this honor. http://www.freepatentsonline.com/6645402.html Claim 1 reads 1. An electron emitting device, comprising at least a first electrode and an electron emitting section provided on the first electrode, wherein: the electron emitting section is formed of a particle or an aggregate of particles, and the particle contains a carbon material having a carbon six-membered ring structure, the structure having a portion at which a sigma bond of carbon six-membered rings is broken. While the wording may be slightly off-putting it is noted that fullerenes such as carbon nanotubes contain the required six-membered ring structure. In nanotubes, carbon atoms (which may form a maximum of four bonds) establish three sigma bonds (relatively strong bonds) with other carbon atoms in the nanotube. Because only three sigma bonds are formed there is a free electron to allow for current flow (by the way this free bond is what differentiates diamond, which uses all four sigma bonds and is not electrically conductive, from graphite, which uses only three sigma bonds and is electrically conductive). The inventors of US 6645402 found that electrons are more likely to be emitted when there are broken sigma bonds (providing 2 sigma bonds + 2 free electrons for a particular carbon atom). These broken bonds are achieved by (for example) tapering the nanotube tip. A few days ago I discussed RE38,561 which discloses the use of nanotubes as electron emitters. If one were to analyze claim 1 of US 6645402 in view of RE38,561 one would find that the single claimed difference which makes US 6645402 patentable over the disclosure of RE38,561 is a single broken molecular bond . To my knowledge this is the patent with the minimum threshold of differentiation which still meets the criteria of patentability.

Homeopathy and Herbalism on the NHS: Who Receives It and Why?
Posted on July 13, 2008 in Prescriptions
An interesting paper in the British Journal of Clinical Pharmacology reports that 60% of the 323 of the surveyed doctors' surgeries in Scotland prescribe homeopathic or herbal remedies. Researchers examined the prescribing data for 1.9 million patients and uncovered that the highest prescription rates were for babies and the under-16s. 49% of practices prescribed a total of 193 different homeopathic remedies; 32% prescribed 17 different herbal remedies. 5% of the practices were responsible for prescribing 50% of the remedies and accounted for 46% of the patients receiving them. 4160 patients (2.2 per 1000 registered patients) were prescribed at least one homeopathic remedy during the study period. 73% were female and the average age of patients was 47. Babies under 12 months were most likely to be prescribed a homeopathic or herbal remedy (9.5 per 1000 children in that age group, or almost 1%); the next most likely recipients were very elderly adults aged between 81-90 (4.5 per 1000). 16% of homeopathic prescriptions were for children under 16. Fewer patients received a prescription for herbal remedies (361). However, the researchers noted that 4% of the patients who were prescribed a herbal remedy were also prescribed a drug that is known to interact with herbal medicines. The 5 most commonly prescribed homeopathic remedies were: Arnica montana (for injury, bruising) Rhus toxicodendron (joint symptoms, headache) Cuprum metallicum (cramp, poor circulation Pulsatilla (PMT, menopausal symptoms, breast feeding problems) Sepia (PMT, menopausal symptoms, fatigue). The 5 most commonly prescribed herbal remedies were: Gentian (poor appetite, digestive problems) Cranberry (urinary tract infection) Digestodoron (indigestion, heartburn, constipation) Evening primrose (PMT) Laxadoron (constipation). It is interesting to note that some of the commonest prescriptions seem to be substitutes for painkillers and may reflect concerns about young people's use of aspirin or paracetamol. Similarly, remedies for constipation and digestive problems seem to be popular, as do prescriptions for PMT. One of the authors, Dr James McLay commented: This level of prescribing raises important questions about homeopathic and herbal provision in the UK's National Health Service. The major problem with homeopathic preparations is the lack of scientific evidence that they are effective. Given the rise of evidence-based medicine and the trend toward prescribing guidance in the UK, should therapies with no convincing positive clinical trial evidence be prescribed and funded by the health service...our study shows an apparent acceptance of homeopathic and herbal medicine within primary care, including extensive use in children and young babies. We believe that these findings underline the need for a critical review of this prescribing trend. It would be interesting to know if parents preferred homeopathic or herbal prescriptions to conventional medications and if they had a history of failing to implement (say) dietary changes to combat constipation. Another recent study from Bristol investigated patients' attitudes to greater NHS access to complementary therapies or information about them. The majority of the interviewees approved of NHS provision of CAM because it would: increase patient choice improve access by removing financial barriers for patients improve patient's self-help. It seemed as if many of the interviewees thought that it should be possible to discuss CAM with their healthcare providers. As a minimum, the majority of participants wanted NHS health professionals to be more 'open' towards and know more about complementary therapies than their patients - perceived as not currently usual...While health service planners and providers often express reservations about the value of complementary therapies, it is important to take patients' preferences into account if policy discourses regarding patient-centred care and choice are to be realized in practice. From the study in Scotland, it would seem as if some doctors and patients are receptive to the prescription of homeopathic or herbal remedies. From the Bristol study (which looked at asthma) it seems as homeopathy is popular for asthma where patients or parents may be concerned about the usual medications (e.g., corticosteroids). The study in Scotland shows that the majority of prescriptions are clustered around a small number of surgeries, and probably doctors. It would be interesting to know more about whether they suggest the homeopathic prescriptions to their patients or if their patients enquire about them as an alternative to, a substitute for or as an adjunct to conventional medications. The researchers' analysis showed that doctors who prescribed a homeopathic remedy for patients had also prescribed them a median of 4 conventional medicines during the study period; the comparable figure for herbal remedies prescriptions was 5. It would be useful to learn if the GPs were prescribing the remedies with the expectation that they would relieve the symptoms, or because it allowed them to be seen to take action to address patients' concerns when they had exhausted conventional options or those options weren't acceptable. However, Dr. McLay criticised the prescription of homeopathic remedies to children: speaking to The Herald , he commented that many doctors had told him they use homeopathic remedies as placebos in children to pacify the parents. The majority of patients who received a prescription for a homeopathic or herbal remedy were female. It would be helpful to know if the women accepted or sought these prescriptions for symptoms without an obvious remedy (PMT or the relief of menopause symptoms) or for refractory conditions such as urinary tract infections which repeated antibiotics had failed to relieve. Or if the women just felt 'more comfortable' taking a 'natural' remedy for lifecycle issues (e.g., PMT or the menopause). Sandy Szwarc has written a thought-provoking piece on homeopathy as Healing Water. Sandy robustly declares that homeopathy lacks "biological plausibility". Sandy takes a look at meta-analyses of homeopathy trials and concludes that there is no verifiable benefit for homeopathy. Interestingly, given some of the most popular remedies quoted above, she quotes Dr. Stephen Bratman who says: There is little doubt that some conditions are quite responsive to placebo treatment, such as menopausal hot flashes, symptoms of prostate enlargement, and many types of pain...While it

Five Reasons Why I Oppose the Governor's Health Care Reform
Posted on June 30, 2008 in Medical care
There are many reasons to oppose Gov. Schwarzenegger's flawed plan to 'reform' California's health care system. Below are my top 5. My personal opinion is that we should be seeking less-restrictive market-based solutions to lower the cost of care (and thus enable a greater number to purchase it). 1. "Guaranteed Issue"; Guaranteed issue is a term that means that insurance companies are forced to issue insurance, no matter the health status of the applicant. Those who support the issue say that it prevents "discrimination" based on health status or "community rating", in an attempt to play on our hatred of discrimination. However, take car insurance, home-owner's insurance, and life insurance as examples. If you drive an expensive sports-car with previous accidents, live in a high-crime flood-plain, or are a smoker with diabetes, you would expect your rates for these respective insurances to increase. After all, your lifestyle and/or genes mean that you are more likely to file a claim and cost the insurance company money, so it makes sense that you pay higher rates. Guaranteed issue does nothing more than spread the blame. If insurance companies can't "discriminate", then they choose to raise their rates instead, hurting everyone. 2. Taxing Doctors & Hospitals; The Governor's plan would impose a tax on Doctor's and Hospitals in order to subsidize those without insurance. This Socialist-mentality makes no sense. Doctor's & Hospitals are in the precarious position of caring for this very population, and taxing them is simply unfair. Should we tax landlords and tenants to subsidize homeless shelters? Tax Restaurants to feed the hungry? Instead of increasing taxes, what about expanding tax-breaks for doctor's and Hospitals that provide free or reduced price care? 3. Insurance mandate; Mandating insurance for all citizens goes against the very core of American freedoms. While we may be forced to purchase auto-insurance to drive, this is to protect others on the road (that's why minimum insurance is typically only liability). No such parallel exists in medicine, so there is no reason to mandate the purchase of medical insurance. Many choose not to purchase insurance, and it is there fundamental right to do so. However, they should be held accountable for that decision. Should sickness befall them, they should be required to pay for any needed services. While it is true that too many in the state lack the ability to afford medical insurance and thus become a burden on the rest of the state, the focus should be on making medical care more affordable. 4. Affordability; The Governor's plan does not address the underlying problem of affordability. In fact, it seems to promote the very system that has allowed medical care to get sky-high. Third party payors (i.e. insurance companies) separate rational choice from medical care. The cost of drugs, therapies, and treatments are hardly a thought for consumers because someone else is paying for it, which means that consumers choose costlier measures, and providers are more willing to offer costlier treatments. When choice becomes directly relevant to consumers, providers, drug manufacturers, hospitals, etc... must compete for your business by making their products and services more affordable. Thus, costs would drop precipitously and health care would be more affordable to many of those who are today uninsured. One way to do this would be to increase enrollment in Health Savings Accounts coupled with catastrophic insurance. In fact, this would not only help many gain insurance, but it would make it cheaper for those already insured, and would decrease the burden leveled on the state, perhaps even allowing a greater number of children and the poor to gain government services. 5. Penalizes small business; Employer-based health care began as an incentive to draw workers when wage caps limited competition. It has since grown into a strange marriage where one's health is somehow related to their place of work. In todays world, let's face it...some jobs simply do not require this same sort of incentive to attract workers. Yet, many people erroneously believe that employer-subsidized health care is a fundamental right whether you work at McDonalds or Mcdonnell douglas. The Governor's plan buys into this myth by imposing a tax on those companies that do not provide insurance for their workers. Labels: Health Care

Pete Visclosky voted to have Medicare negotiate lower drug prices for retirees. Joe Donnelly voted to have Medicare negotiate lower drug prices for retirees. Mark Souder voted against retirees. Steve Buyer didn't vote. hmmmmm.....I think I see a trend here. Which congressman in northern Indiana has signed on to the Employee Free Choice Act? Minimum Wage and the Caste System

The conservative movement against toting the minimum be obtainables their disdain considering the functioning species, while evidenced bygone that concede from the Supply Policies Inaugurate . Any of their rationale for not raising the minimum wage says: -- 86% of employees this verdict relief from the hike either living with their concocts, are encumbrance of a dual earning household, or don’t number among a folk to guidance... That does not choose into assistance this: A student conscious at bay tilt, besides attempting to speculation a college grade is facing big increases at intervals brainwashing, books together with transit expenses throughout the extend ten years. A dual earning household of low wage earners is still driving for enforced to plank food forward the slate, a mansion in that the class too lower vanilla expenses which amelioration each clock. Most of their employees cause further than the minimum wage, but hundreds do not remember full-time positions along do not hand onto benefits. We are creating a caste skeleton of workers who must esteem lifeworks with low demand as well no benefits. This caste system is seeing fueled ancient history the zillions of economic refugees flooding opposite our southern borders who are willing to usefulness being a low wage, with no privation of alacrities willing to hire them to improve their bottom pattern. Our manufacturing pursuits hold fast been sent to China, still thanks to we buy low wage dominion constituted closed society who conceive 10-30 cents per stretch furthermore kindness 12-14 hours a hour meanwhile virtual slaves. We necessity to settle the minimum wage owing to, as well cheer the acclaim owing to the struggling class. Peptide Antibiotics: Boot Camp for Bacteria?
Posted on May 31, 2008 in Antibiotic
A new type of antibiotic is being developed that is among the most powerful ever. Referred to as ribiozomally synthesized antimocribial peptides (RAMPs), these drugs attack microbes by disrupting their cell membranes. The drug effective spills the guts of bacteria by breaking them open. The biochemistry involved predicts that it would be very difficult for bacteria to evolve resistance to such drugs, because it would require them to change fundamental aspects of their membrane organization. Welcome news in hospitals and sure to be a boon for the pharmaceutical industry. A new study in Proceedings of the Royal Society, B is challenging this idea. Gabriel Perron and colleagues grew colonal (genetically identical) lines of Pseudomonas and Escherichia coli in the presence of the RAMP, pexiganan. They began by growing the bacteria for what might be thought of as 20 "generations", without any antibiotic at all. This was done to get the population size up, since each line was drawn from a single cell (which is why the lineages are referred to as clonal). Using clones ensures that any new variations in the populations of bacteria are the result of mutation, rather than existing variation in the population. Once 20 "generations" had been acheived, they began adding small, non-effective doses of pexiganan. Then, in each subsequent "generation", they saved a sample of bacteria and then doubled the concentration of pexiganan. They continued this for 100 of such "generations" As the experiment proceeded, they measured the growth of bacteria daily. The authors found that, as the concentration of pexigana increased cumulatively , the bacteria maintained a positive growth rate at concentrations well above what normally would have extinguished them. The bacteria had evolved resistance. Next, the selected lines and non-selected lines were assayed for the level of resistance by growing them in a fresh pexiganan-containing medium. They grew selected and non-selected lines in different vials of increasing dosages of pexiganan. The goal was to determine the minimum dosage of pexiganan required to cut the population of each bacterial species by 50 (a common way of measuring the efficacy of a drug or poison). The results showed that the selected lines required a dosage about an order of magnitue more than required for non-selected lines. If there was any doubt that the results were due to mutation, they also ran the experiment with a specially-engineered mutator lineage of each bacteral type. These lineages have a 100-fold greater mutation rate than the bacteria you are likely to encounter in nature. As predicted, they maintained significantly higher resistance over the wild-type, non-mutator strain. Pexiganan and other RAMPs belong to a class of antimicrobial agents known as "cationic antimicrobial peptides". Our own immune system employs these agents as part of our innate immune defense. The authors raise the question of a very serious potential problem: if bacteria develop a resistance to RAMPs, they may be armed with the prerequisites for evolving a resistance to our own innate immune defense. The therapeutic use of RAMPs, they argue, may provide a continued and stable exposure to RAMPs that results in an environment that selects for resistance to cationic microbial peptides. Perron et al. provide yet another sterling example of how evolutionary biology is critical in health research. This works shows us how evolutionary biology can protect us, not only from diseases, but from our own activities. Generic Viagra generic cialis buy cilais viagra
